John is the owner of Point Forward Physical Therapy. John graduated
from Stevens Point Area Senior High in 1990. After finishing a
bachelors degree in biology at Marquette University he enrolled in the
Masters in physical therapy program. Upon graduating from school in
1997 he began his career as a traveling physical therapist working in
settings including outpatient orthopedics, inpatient hospital care,
inpatient rehab, and long term care. After taking time off to tour Europe
he returned to Wisconsin and worked rural outpatient clinic in southeast
Wisconsin. He returned to the central Wisconsin area in 1999 when he
began working as a staff physical therapist at Saint Michaels Hospital.
In 2001 John was the primary Physical Therapist for the Satellite clinic
in the Plover Family Practice building. John was later promoted and
became the Coordinator of Rehab services for the Plover Clinic.
Over the past 10 years John has had many great experiences that he
feels have influenced him and directly benefit his patients. He has
attended continuing education course including treatment techniques for
the foot and ankle, knee, hip, low back, thoracic spine, shoulder and
elbow. His treatment of patients is heavily based in the most current
research. John also has served as adjunct faculty at UW-Stevens Point.
In the spring of 2007, John founded Point Forward Physical Therapy
Ltd. He is excited that this new endeavour will allow him to focus on
patient care and treat without any barriers. By caring for his patients in
the Adventure 212 facility he is now able to care for their needs in a
facility which has no equal in the central Wisconsin area.

Sheila Banovetz is an Outpatient Physical Therapist at Point
Forward Physical Therapy and an Adjunct Associate Professor at
the University of Wisconsin, Stevens Point. She obtained a degree
in Physical Therapy from Washington University in St Louis in 1985.
She has practiced as a physical therapist for 23 years in Chicago,
Columbia, MO and served on the Physical Therapy faculty at the
University of Iowa from 1995-1996 before she moved to Stevens
Point. She works primarily with clients with muscle and movement
impairment syndromes.
Sheila Banovetz MSPT
Damon grew up in Port Edwards, WI and graduated from Assumption
High School (Wisconsin Rapids) in 1990. He attended Marquette
University’s program in Physical Therapy and earned a Bachelor’s
degree of Science in Physical Therapy in 1994. After graduation from
Marquette, he accepted his first position as a staff physical therapist
at St. Michael’s Hospital, where he worked for 14 years. During his
time at St. Michael’s Hospital, Damon developed a specialization in
rehabilitation of the foot/ankle. Because of his work to develop a
program to treat the foot/ankle, Damon received Ministry Health Care’
s Circle of Excellence Award and was nominated for this award a
second time.
Damon has accumulated a significant resume of continuing education
courses since graduation. Besides the foot/ankle, he has attended
courses in the following: gait studies, neck, shoulder, thoracic spine,
lumbar spine, pelvis/sacrum, hip, knee, elbow, manual therapy
techniques, surface EMG, modalities, ethics in healthcare, clinical
diagnostic imaging, neuromuscular re-education, and updates on
surgical techniques. He has performed lectures for the University of
Wisconsin Stevens Point in the areas of gait and foot/ankle
evaluation and treatment.
Damon Kozicki PT
Caitlin graduated from Beaver Dam High School and went on for her
degree in exercise science from Marquette University. Caitlin then
earned her doctorate of physical therapy degree from Marquette
University in 2006. She took her first position as a staff physical
therapist at the Cleveland Clinic in Cleveland, OH. Caitlin then went
on to work as a traveling physical therapist which allowed her to
experience a variety of settings including out-patient orthopedics, and
in-patient physical therapy.
Caitlin has attended continuing education courses in neurological
rehabilitation, ethics, modalities including ultrasound and electrical
stimulation, work conditioning, back pain and vestibular and balance
disorders.
Point Forward Physical Therapy is excited about the new dimension
